We are seeking applicants for our full-time Community Transition Specialist and part-time Youth Leadership Advocate. We're looking for candidates with a strong understanding of disability rights, independent living philosophy and experience working with people with disabilities. People with disabilities encouraged to apply.
The Community Transition Specialist will assist persons with disabilities, 59 and younger, to transition from nursing homes into their own homes in the community.
The Youth Leadership Advocate will teach youth with disabilities self-advocacy and pre-employment transition skills in high schools throughout McHenry County.
